Health Consumers NSW and Cancer Voices NSW will be jointly presenting a workshop on consumer leadership at the Consumers’ Health Forum Australia Summit 2021: Shifting Gears on 18-19 March 2021.
As a consumer leader you are invited to respond to this survey to provide the perspectives of someone who has worked in, and has an understanding of, consumer leadership in the broader health and more specific cancer sectors. Your input will help ensure that the workshop content is current and informative for those participating.
We are seeking information about your experience as a consumer leader:
Why and where you started
Leadership attributes
Your successes as a consumer leader
One piece of advice to future consumer leaders.
We plan to approach survey respondents for short sixty second filmed interviews for use during the workshop. You can indicate your willingness to participate in these at the end of the survey.
Details collected through the survey will be anonymous unless you are interested in being interviewed.
Survey participants will have the chance to opt-in to a draw for receipt of one of three $100 gift cards.
The survey will take about 5-10 minutes to complete. The survey will remain open until 3 March 2021.

Cancer Voices NSW
Cancer Voices NSW provides the independent voice of people affected by cancer in NSW, working to improve the cancer experience of the over 49,000 people who, according to official projections, will be diagnosed each year. Established in 2000, we focus on the areas of diagnosis, information, treatment, research, support and care, survivorship and policy. To achieve this, we work in partnership with decision makers and providers of these services, ensuring the patient perspective is heard, from planning to delivery.
